a difference?As a Forensic Case Manager, you will play a vital role
in our mission to address the bio-psychosocial needs of our
patients. This position is responsible for managing the case of
individuals involved in the court system who are receiving mental
health treatment, coordinating with courts and legal entities,
developing individual treatment plans, and ensuring compliance with
legal requirements while facilitating their recovery and reducing
recidivism rates.We are seeking a dedicated individual who can
focus on conducting comprehensive assessments, linking patients to
community resources, advocating for their needs, and maintaining
detailed documentation of their progress within the legal
framework.Conduct through evaluations of new patients, including
behavioral history, mental health needs, and risk assessments to
develop appropriate treatment plans.Create individualized treatment
plans based on legal mandates, court orders, and clinical
assessments, collaborating with the treatment team to ensure proper
interventions.Monitor patient progress, coordinate with other
healthcare providers, and advocate for necessary services to
support their treatment and community reintegration.Maintain
regular communication with courts, probation officers, and
attorneys to provide updates on patient progress and address
underlying issues contributing to their behavior.Regularly assess
patients for potential risks of violence or self-harm, implementing
safety measures and reporting concerns to relevant parties.Maintain
detailed and accurate records of patient interactions, including
treatment progress, clinical assessments, and legal updates,
ensuring compliance with HIPAA and other regulations such as Joint
Commission.Proven experience in developing and managing treatment
plans, coordinating care, and advocating for patient needs.Your
Experience Matters What were looking for:Education & Education:
Bachelor's Degree from an accredited college or university in
Social Work, Psychology, or related field with a focus on forensic
mental health. Master's Degree preferred.Experience: A minimum of
two (2) years experience in a psychiatric health care facility,
with a strong focus on forensic psychology is essential; crisis
intervention, and treatment skills; and strong clinical assessment
skills preferred. Joint Commission experience is
preferredLicensure: Must possess an Illinois State license as a
mental health professional (L.C.P. or L.C.S.W.)Additional
Requirements: May be required to work flexible hours and

compensation range is $52,000 - $60,000 annually with a $2,000
